Incident Management
· Oversight and monitoring of agency incident reporting
· Assigning and reviewing all investigations of reportable incident and notable occurrences
Interface with outside monitoring agencies including OPWDD, DOH and the NYS Justice Center
Chair for the organization’s Incident Review Committee (IRC).
Maintain OPWDD Investigator certification and conduct investigations, as needed
Quality Enhancement
· Draft Plans of Corrective Action for OPWDD audits
· Coordination and monitoring of agency internal reviews and quality visits by Quality Assurance Specialists
Compile and prepare data for analysis for Annual Trend Report for incidents, fire drills and other areas
QUALIFICATIONS:
· BA or BS in a Human Services field and minimum of three years’ experience in QA Functions
Expert knowledge of OPWDD, NYS Justice Center and NYS DOH governing regulations and OPWDD IRMA Software and Justice Center WSIR software
