The Center for Wellbeing and Happiness is seeking 3 Community Health Workers to join the team! The Center for Wellbeing and Happiness (CWBH) is an initiative of the Lower East Side Girls Club (LESGC) which provides free wellness programs to Lower East Side community members of all generations and genders. Reporting to the CWBH Director, Community Health Workers will build individual and community capacity to address health and wellbeing, and serve as a critical link between the community and the Center. They will assist community members in connecting to the services they need, give informal counseling and guidance, advocate for individual and community health needs, offer interpretation and translation services, and provide culturally appropriate health education and information.  

 

Responsibilities

Coaching and Support

  • Share encouragement to support members on their journey to strengthen their health and wellbeing

  • Promote goal setting by providing holistic assessments and co-developing member care plans, which outline their strengths and self-defined goals, steps toward achieving the goals, available supports, and resources needed

  • Build trust and motivate members to accomplish their self-determined goals

  • Provide ongoing social support and individual counseling in person, by phone, and in writing

  • Schedule follow up appointments to track progress on care plans

  • Collect and share feedback on ways to better support members

 

Service Coordination

  • Assist members in navigating healthcare and social service systems

  • Build the capacity of members to access resources specific to their needs, including health insurance, food, housing, quality care, and health information

  • Implement member care plans by: 

    • Enrolling members in CWBH services

    • Providing referrals to coordinating partner organizations and City agencies in various areas, including, but not limited to: healthcare, housing assistance, academic enrichment, family budgeting and consumer education, nutrition, counseling, job-readiness, recreational services, business development;

    • Screening and enrolling members in benefits such as Medicaid; Special Supplemental Nutrition Program for Women, Infants and Children (WIC); Supplemental Nutrition Assistance Program (SNAP)

  • Continuously expand knowledge and understanding of community resources and services

  • Regularly reviews and compiles community resources to help support participants in meeting their goals

  • Coordinates monthly and quarterly meetings with coordinating service providers

 

Community Engagement & Educational Empowerment

  • Regularly staff the CWBH front desk–checking members in, directing members to classes, and promoting the Center’s services

  • Lead new member orientation sessions 

  • Organize, attend, and/or present at community meetings, events, and health fairs

  • Advocate for local health needs with government agencies and service providers

  • Respond to public and community inquiries about organizational initiatives

  • Share and post outreach materials such as flyers, posters, and other promotional materials in the community

  • Support the co-design of marketing and recruitment strategies with families and community members

  • Provide translation and interpretation for members, as needed

 

Administration & Reporting

  • Maintain case notes and other supporting documentation to validate enrollment, benchmark, and outcome achievement

  • Project and report on enrollment, benchmark, and outcome data 

  • Support the development of CWBH policies and procedures 

  • Make home visits and organize transportation for their members to referral appointments, as needed

 

Education and Experience

  • Has one of the following: 

    • (1) An associate’s degree in a human services field or current enrollment toward such degree and at least two years of successful experience within the last five years providing access services and experience working with diverse low-income populations or 

    • (2) At least five years of successful experience providing access services and experience working with diverse low-income populations.

  • Understanding of issues of equity and social justice

  • Experience integrating participant voice into programming

  • Fluent in English and has a professional working proficiency in Spanish, Cantonese, Mandarin, or the Fuzhou dialect

  • Preferred: Experience delivering services to low-income families in the Lower East Side/Chinatown; have a Family Development Credential

  • Belief in the mission and values of LESGC 

  • Community – ability to make accurate community needs assessments and has deep roots or shared life experiences with Lower East Siders

  • Capacity building – ability to support communities and individuals seeking to improve their health and wellbeing

  • Communication – ability to listen, speak, and write in English plus ability to listen, speak, and write in Spanish, Cantonese, Mandarin, or Fujianese

  • Confidentiality – ability to keep members’ health personal information matters private

  • Interpersonal – friendly and sociable but able to set appropriate boundaries; builds trust and provides encouragement to members.

  • Organization – ability to set goals and develop an action plan, manage time, keep records, to set priorities and manage multiple projects simultaneously, to delegate wisely, and to respond flexibly to changes

  • Coordination – ability to identify and access social services and health-related resources; to network and build coalitions; to make and follow-up on referrals

  • Facilitation – ability to share information, to respond to questions, to reinforce ideas, and to adapt methods to various audiences

  • Problem solving – ability to find creative ways to implement programs or to stretch resources; ability to break down a complex challenge into smaller, more manageable issues. 

  • Cooperation – ability to establish positive, supportive relationships with participants, colleagues, and partners from diverse cultural and socio-economic backgrounds.

  • Familiarity with strengths-based approaches, self-advocacy strategies, DYCD’s Circles of Support family engagement framework, positive youth development, and social emotional learning principles

  • Knowledge of New York City agencies and programs

Organizational Relationships: 

  • Reporting to the CWBH Director

  • Partnering with the CWBH Program Manager, Instructors

 

Must be available some nights and weekends; this position needs to be available to work either Tuesday-Saturday, or Monday-Friday for an 8 hour shift between 8am and 9pm

 

Salary & Benefits:

The salary for this position is $50,000 per year. LESGC offers medical, dental, and vision insurance. The organization contributes a generous amount towards the employee’s medical premium, while the dental and vision insurance premiums are paid by the employee. A Health Reimbursement Account is offered as a supplement to our medical insurance plans to help offset out-of-pocket medical expenses. About the Center for Wellbeing and Happiness 

Rooted in the belief that the wellbeing of our youth members is intimately connected to the wellbeing of their family, their community, and their world, the Girls Club has expanded its mission and service population with the launch of their Center for Wellbeing and Happiness (CWBH).

The Center for Wellbeing and Happiness strategically addresses the wellbeing and health disparities in the Lower East Side by utilizing intentional, trauma-informed, holistic programming rooted in community-centered resilience, prevention, recovery, and self-reliance. The CWBH will serve as a wellness hub providing the space and opportunity for local organizations, expert practitioners, health-based city agencies and institutions, and our talented and resourced- filled community members, to connect, collaborate, and serve. Together, we will create programming that addresses the totality of our community’s wellbeing needs.

In fall 2020,  the Girls Club launched our Virtual CWBH to provide free virtual wellness programs in the midst of the pandemic as we awaited the renovation of the physical CWBH. In fall 2022, we will open the doors to CWBH: a 5,000 ft2 state of the art space, adjacent to the existing Girls Club facility.
