Job Title: Family Coach NY (PT)

 

Status:                 Part-Time Exempt Position (20 hours per week)

 

Report to:           Family Services Manager

 

 

 

Job Summary: Responsible for coaching participants on Campus to achieve self-reliance, improved life skills, and empowerment. Responsibilities include: providing the primary source of support and encouragement, tracking the participants’ progress toward education and employment goals, assisting with personal student goal setting, resource referral, crisis intervention, and collecting outcome data in collaboration with the Family Services Manager. The coach works directly with participants to monitor program partnership and inspires personal success.

Primary Responsibilities:

 A.     Direct Services

 o   Co-develop individual goal plans for participants to provoke self-discovery and personal growth; engage in goal setting and meaningful action.

 o   Coach participants by encouraging self-responsibility, promoting alignment between their personal values and actions and generating innovative solutions using empowerment techniques.

 o   Coach participants to progress in their life skill development and achieve goals by providing support, crisis intervention, guidance, and resource referral. Reinforce Life Skills lesson content/topics during coaching sessions.

 o   Collaborate and strategize with participants on effectively partnering with outside supportive systems and agencies.

 o   Support participants in managing their financial to ensure they are maximizing their partnerships.

 o   Maintain efficient, confidential and organized records; provide timely and appropriate case notes in apricot.

 o   Collect, record and administer participant self-assessments and other participant data regarding outcomes progress.

 o   Administer random UA’s as necessary.

 o   Collaborate and participate in team meetings with program staff.

 o   Work with families and other staff collaboratively through family goal planning sessions.

 B.     Program Support

 o   Work closely with the Family Services Manager to ensure outcomes are achieved.

 o   Collaborate with other employees to ensure quality, seamless service delivery.

 o   Collaborate with Resident Council Leaders and Volunteer Coordinator to support community partnerships and events.

 o   Maintain professional conduct at all times, in conjunction with The Jeremiah Program mission, and contribute to the inclusive and safe learning environment for the community.

 o   Assist participants with immigration related issues and advise on immigration specific resources.

 C.     Other Duties as assigned

 o   All Jeremiah Program employees are mandated reporters and are legally required to ensure a report is made when abuse is observed or suspected.

Qualifications:

 ·         BA/BS in education, human services, social work or related field and 3-4 years’ experience working with diverse populations.

 ·         Significant experience in the areas of project management and student case management.

 ·         Thorough knowledge of education opportunities and community resources including county resources, benefits, and assistance programs.

 ·         Experience accessing and advising on immigration related resources

 ·         Excellent written and verbal communication skills.

 ·         Detail orientated and organized.

 ·         Ability to work in a team environment, interact with management, direct service staff and participants by delivering high-quality service.

 ·         Ability to problem-solve, multi-task, complete work on time and work independently.

 ·         Proficiency in computer skills, i.e. Microsoft, Sharepoint and ability to learn Apricot database.

 ·         Passion for the Jeremiah Program mission and values.
